  2. (Norvasc)AMLODIPINE BESYLATE: 10MG nightly night

  3. (Zoloft) SERTRALINE: 100MG 3 a day morning

  4. (Ritalin) METHYLPHENIDATE: 20MG 4 times a day; morning then every 4 hours

  5. (Flomax)TAMSULOSIN: 0.4MG twice a day morning and night

  6. (Zocor)SIMVASTATIN: 20MG once a day at bedtime night

  7. (Detrol LA)TOLTERODINE: 4MG One capsule per day for uinary urgency/frequency night

  8. (Proscar)FINASTERIDE: 5MG night

  9. (Sonata)ZALEPON: CAP 10MG night

  10. (Trimix) injectable three-drug prescribed medication used to treat erectile dysfunction: Increase dosage as instructed Use up to 3 times per week

    1. Prostaglandin E1 (PGE1), also known as alprostadil

  11. PREVIDENT 5000 BOOSTER GEL: 500ML Tooth past with fluoride

    On 04/11/2017, refilled this perscription with G.U.M Paroex, CHORHEXIDINE GLUONATE ORAL RINSE USP, 0.12%, 1 Pint(473ml)

NON-Prescriptions / Supplements

  1. Claritin Taken once a day before bed to minimize snoring. Sometimes substitute 50 mg of Benadryl (Diphenhydramine HCL)

  2. Asprin: 81MG, 1 pill morning and night

  3. Vitamin D: 1000 I.U. Two in the AM

  4. Gas-X: 375MB (3 to 6 softgels); as needed

  5. Beano: 375MB (4 to 12 softgels); as needed

  6. Niacin: 500MG taken daily at night

  7. Acid reducer:150MB twice a day

  8. Prilosec: 20MG twice a day

  9. Naproxen Sodium 220MG 1 tablet morning and night. Will switch to Ibuprofen tablets: 200MG 4 tablets morning and night in the future. On 08/11/2017, I switched to Ibuprofen. Then on 09/04/2017, I went baack to Naproxen, because the Ibuprofen was irrtating my stomach. The irration stopped, and I have continued taking Naproxen.
